Introduction
Hypertension in the United States is very prevalent due to personal characteristics, genetics, and health disparities. Hypertension is one of the leading causes of cardiovascular disease morbidity and mortality. “Hypertension refers to a sustained condition of elevation of blood pressure within the arterial circuit,” (T. L Norris, 2019, p. 56). It is also characterized by having a blood pressure that is greater than or equal to 130/80. “Hypertension can result from either primary or secondary causes,” with primary hypertension being caused by an unknown origin, and secondary hypertension being caused by a known cause that can be identified and corrected, (S.L Lewis, L. Bucher, M.M Heitkemper, M.M Harding, 2017). Although all races suffer from hypertension in America, African Americans have the highest burden when it comes to cardiovascular disease and hypertension. In an article titled “Hypertension in African Americans: Advances in community outreach and public health approaches, the researchers stated, “multiple factors have contributed to this major public health crisis, including medication adherence, racial inequities in diagnosis and treatment, lack of culturally competent care, and disparities in healthcare access,” (D.P. Ferdinand, K.C Ferdinand, S. Nedunchezhian, 2020).
With African Americans being the highest race to experience hypertension in their communities, researchers have focused on a community-based approach to increase the participation of African Americans in risk factor and disease reduction. They also want to focus on reducing and eventually eliminating the risk factors and health disparities that contribute to the high prevalence of the disease. Some of the many risk factors that contribute to the high African American percentage of hypertension are “high BP, high cholesterol, diabetes, and maintaining healthy lifestyle behaviors,” with the main focus being on maintaining health lifestyle behaviors, (D.P. Ferdinand, K.C Ferdinand, S. Nedunchezhian, 2020). As long as control of these risk factors is coupled with the use of medication and treatment, the United States should a decrease in the number of people with hypertension in this country. Medication compliance and adherence is another risk factor that plays a major role in the high amount of African Americans that experience hypertension and cardiovascular disease. The lack of adherence to anti-hypertension medication is influenced by socioeconomic status, health systems in the country, medical conditions and patient characteristics. The researchers stated that, “63% of black HTN patients exhibited inadequate control, with 81% of these patients even less likely to adhere to anti-HTN medications,” (D.P. Ferdinand, K.C Ferdinand, S. Nedunchezhian, 2020). Lastly another risk factor that predisposes African Americans to hypertension is the lack of access to healthcare. This social determinant may the most important, because it is the easiest to change. African Americans face a lack of insurance coverage, and high costs of health care. There is currently an increased need for public health interventions, and representation in healthcare for minorities. All in all, there needs to be a change in research and approach when it comes to improving the healthcare management of hypertension within the African American population. This will help decrease the cases of hypertension in the United States, for not only the African American community, but the entire nation as well. The purpose of this study is to change the African American Community’s attitude toward hypertension, better their behaviors to decrease the number of people with hypertension in the African American population, and to educate on the severity of hypertension. My main question is, can further educating the African American community on hypertension, and its severity help future African American’s avoid developing hypertension and cardiovascular disease as a population.
Population
My population will consist of 200 African American men and women totaling out to 400 people and all of the patients must be residents in Texas. The criteria for participation in my study will be; patients within the ages of 45-55, must at least have stage 1 hypertension, and must not have health insurance. The location I will use to recruit the participants for my sample will be in inner Houston, Texas. The recruitment will be conducted by the criteria stated above along with advertisements used to attract people that meet the criteria.
Ethics
For the ethics portion of the paper, I will focus on obtaining consent from the participants. Informed consent is the most important portion of the study. You want to obtain this as a researcher in order to prevent lawsuits, or risks to the participants. I would make all 400 patients in my sample sign an informed consent form, and then supply a copy for all the participants. The informed consent form would then provide all the guidelines, procedures and reason for the study. The patients will also be able to decline participating in the study initially, and cancel their participation during the study at any time. Lastly I will make it known that the study is approved by the IRB (Institutional Review Board), and Houston Baptist University’s School of Nursing and Allied Health, to ensure that they know the study was ethically approved.
Plan
This study will be a quantitative research design, specifically a quasi-experimental pre test posttest design. I will provide a series of question before and after a presentation, and video about hypertension. This method will be done to determine the knowledge deficit African American’s have when it comes to hypertension. It will also help determine whether there is a change in attitude and knowledge after the video and presentation. As mentioned before I will collect data through analyzing questionnaires before and after the video, and short lesson on hypertension. This will allow for me as the researcher to see if the group of patients changed they’re attitude toward the importance of hypertension, and learned something new about the disease as a whole. The expected findings for this study would be to see a change in how African American’s view hypertension, their behaviors that may have caused them to develop hypertension, and an increased desire for assistance to prevent or decrease their hypertension.
Findings
The nursing implications that are going to be performed for this project are having seminars and informational sessions on hypertension, to educate the community about the severity of the disease. Another implication that can be done is providing educational opportunities on diet, with the assistance of dieticians, to help encourage African Americans to change their diet choices. High salt diets are on of the main contributors to developing hypertension besides genetics, in the African American community. The expected findings are relevant because they are essential to lowering the percentages of African Americans with hypertension in Texas and the United States.
